Below are forms that may be printed out for your use.

Absentee Excuse Form	- Use this form to notify the school within three days if your child was absent from school.
Authorization for Administration of Medications	- This form is to be completed and returned to allow your child to recieve medication from our school nurse - Mrs. Fordyce.
Building Use Form	- Use this form to request permission to use the NCSD Building and/or Grounds for use from out outside agency.
Bus Form	- Use this form to notify the school if your child will be riding a different bus either in the morning or the afternoon.
Guest Authorization Form	- Use this form if a junior high or high school student would like to bring a student from a different school district to an NCSD function, such as a dance.
Health Certificate / Appraisal Form	- This is to filled out by your child's physicial and brought to our school nurse - Mrs. Fordyce.
